Intensive Daily Treatment When You Need It Most
San Diego PHP: The Highest Level of Outpatient Addiction Care
When outpatient therapy isn’t enough, but you don’t need 24-hour residential treatment, our Partial Hospitalization Program (PHP) bridges that gap. PHP provides intensive, full-day programming with medical oversight and comprehensive therapeutic support—all while allowing you to return home each evening.
At Bonus Round Recovery, PHP isn’t just more hours in a chair. It’s immersive, structured care designed for people who need significant support to break free from addiction. Think of it as residential treatment without sleeping there—the same intensity, the same medical attention, but with the benefit of practicing your recovery in the real world every night.
What is PHP?
Comprehensive Daily Care Without 24/7 Supervision
Partial Hospitalization Program (PHP) is the most intensive level of outpatient addiction treatment available. It provides structured programming throughout most of the day, typically 5-7 days per week, with clinical and medical oversight comparable to residential treatment.
PHP is ideal for:
- Individuals transitioning down from residential treatment or detox
- People who need more support than traditional IOP can provide
- Those requiring daily medical monitoring or psychiatric oversight
- Anyone at high risk for relapse or with recent relapse history
- Individuals with co-occurring mental health disorders needing intensive care
- People without stable support systems who need daily structure
- Those who tried less intensive treatment without success
PHP might not be right if:
- You need 24-hour medical supervision or are medically unstable
- You require inpatient psychiatric hospitalization
- You’re actively in severe withdrawal requiring detox
- Your living environment is unsafe or involves active substance use (consider our transitional housing instead)
If you’re unsure whether you need PHP or IOP, we’ll help you determine the right level of care during your pre-screening call.

Evidence-Based Treatment Modalities
Therapies Proven to Work
Cognitive Behavioral Therapy (CBT) The gold standard for addiction treatment. CBT helps you identify the thought patterns and beliefs that lead to substance use, then teaches you how to change them. You’ll learn to recognize triggers before they become cravings, and develop healthier ways to cope with stress, emotions, and life challenges.
Dialectical Behavior Therapy (DBT) Originally developed for emotional regulation, DBT is incredibly effective for addiction, especially when co-occurring mental health issues are present. You’ll build skills in:
- Mindfulness – Staying present without judgment
- Distress Tolerance – Getting through crisis without using substances
- Emotional Regulation – Managing intense feelings in healthy ways
- Interpersonal Effectiveness – Building and maintaining healthy relationships
Group Process Therapy Daily group sessions where real healing happens. Share your story, hear others’ experiences, challenge your beliefs, receive feedback, and build accountability. The group becomes your recovery family.
Individual Counseling Personal therapy sessions 1-2 times per week focused on your unique history, trauma, goals, and challenges. This is your safe space to work through difficult emotions and experiences that may be fueling your addiction.
Trauma-Informed Care Many people struggling with addiction have underlying trauma. Our trauma-informed approach ensures you feel safe while we gently address painful experiences that may be driving substance use.
Life Skills Education Recovery isn’t just about stopping drugs or alcohol—it’s about building a life worth living. We teach practical skills in:
- Financial management and budgeting
- Employment readiness and interviewing
- Healthy communication and boundaries
- Self-care and wellness practices
- Time management and goal-setting
- Conflict resolution
Family Therapy and Education Addiction affects everyone. We offer family education sessions to help your loved ones understand addiction, support your recovery, and begin healing their own pain.

What Makes Our PHP Different
The Bonus Round Recovery Difference
Founders Who’ve Walked This Path Linnsey and Tony aren’t administrators who learned about addiction from textbooks. They’re recovering addicts who’ve experienced homelessness, hit rock bottom, and clawed their way back to thriving lives. When they tell you recovery is possible, they’re not quoting statistics—they’re sharing lived experience. That authenticity permeates every aspect of our PHP program.
Small Groups, Big Impact With our 8:1 client-to-clinician ratio, you’re not one of 30 people fighting for attention. You’re part of an intimate community where everyone knows your name, your story, and your goals. This isn’t a treatment factory—it’s a family.
Medical and Psychiatric Oversight Many PHP programs lack proper medical support. Not ours. We provide regular medical monitoring, psychiatric evaluation when needed, and medication management for co-occurring conditions. Your physical and mental health matters just as much as your sobriety.
Whole-Person Healing We don’t just focus on substance use. We help you address:
- Physical health and wellness
- Mental health and emotional regulation
- Financial literacy and stability
- Relationship repair and communication skills
- Employment readiness and life skills
- Spiritual wellness (if desired)
- Overall sense of purpose and meaning
Real-World Practice Unlike residential treatment where you’re isolated from real life, PHP allows you to practice your new skills every evening and weekend. You’re learning recovery while simultaneously living it—making the transition after graduation much smoother.
Flexible Admission We admit new clients throughout the week as space allows. You don’t have to wait weeks to start treatment when you’re ready now.
Available 7 Days a Week Monday through Sunday, 8am-8pm. Crisis doesn’t keep business hours, and neither do we.

PHP Combined with Transitional Housing
Complete Immersion in Recovery
For maximum success, many clients combine PHP with our transitional housing. This combination provides:
Seamless Integration – Live, eat, sleep, and breathe recovery for 12 weeks without competing demands or distractions
24/7 Sober Environment – No access to substances, no triggering situations, complete focus on getting better
Peer Support Around the Clock – Your housemates are your treatment peers, providing support during evenings and weekends

Safe San Diego Location – Beautiful, healing environment away from old people, places, and things

- You’re relocating from out of state
- You don’t have stable housing
- You want to give recovery everything you’ve got
Medical Oversight in PHP
Comprehensive Health Support
One of the key advantages of PHP over lower levels of care is access to medical and psychiatric professionals.
What Medical Oversight Includes:
Initial Health Assessment: Comprehensive evaluation of your physical and mental health, medical history, and any conditions requiring monitoring or treatment.
Ongoing Medical Monitoring: Regular check-ins to track vital signs, assess medication effectiveness, monitor withdrawal symptoms, and ensure overall health stability.
Medication Management If you’re taking medications for addiction treatment (like Suboxone, Vivitrol, or naltrexone) or mental health conditions, we’ll monitor effectiveness and side effects, adjusting as needed.
Psychiatric Evaluation and Support. Many people with addiction also struggle with depression, anxiety, PTSD, bipolar disorder, or other mental health conditions. Our program includes psychiatric evaluation and ongoing support for co-occurring disorders.
Crisis Intervention: If a medical or psychiatric crisis arises, we have protocols and staff trained to respond immediately, ensuring your safety at all times.
Coordination with Outside Providers We work with your primary care physician, psychiatrist, or other medical providers to ensure coordinated, comprehensive care.
This level of medical support sets PHP apart from IOP and ensures you’re safe while you heal.
Insurance & Costs for PHP
Investment in Your Future
PHP is more expensive than IOP because it includes:
- Full-day programming (6-8 hours daily vs. 3-4 hours)
- More therapy sessions and clinical contact
- Medical and psychiatric oversight
- Comprehensive case management
- Higher staff-to-client ratios
We Accept:
- Out-of-network private insurance benefits
- Cash pay with flexible payment options
- Currently expanding to in-network providers
Good News About Insurance: Most private insurance plans cover PHP at higher rates than IOP because it’s medically necessary intensive treatment. Many people are surprised to find their out-of-pocket costs for PHP are manageable.
Free Insurance Verification: Not sure what you’re covered for? We’ll verify your benefits at no cost or obligation. Send us your insurance card, and we’ll do the rest.
Transitioning Between Levels of Care
PHP as Part of Your Recovery Continuum
Stepping Down from Residential Treatment If you’ve completed inpatient residential treatment or detox, PHP is often the next step. It maintains intensive support while you transition back to living in the community.
Stepping Up from IOP If IOP isn’t providing enough structure or you’re struggling to stay sober with less intensive care, PHP offers the additional support you need.
Stepping Down to IOP As you progress through our 12-week PHP program and demonstrate stability, we may recommend transitioning to IOP for your final weeks, gradually decreasing intensity while maintaining support.
After PHP Graduation Options include:
- Continuing with weekly outpatient therapy
- Attending community support groups (AA/NA)
- Moving into sober living for continued structure
- Returning home with strong aftercare plan
We’ll help you determine the right next step based on your progress, stability, and individual needs.
After PHP: Your Continued Journey
Setting You Up for Long-Term Success
Graduation from our 12-week PHP program is a major milestone—and we’ll celebrate it with you. But we also know recovery is a lifelong journey, not a destination. We prepare you for what comes next.
Comprehensive Aftercare Planning:
- Connections to local AA and NA meetings
- Referrals to ongoing therapists and psychiatrists
- Sober living recommendations if you need continued structure
- Alumni support group to stay connected with your recovery community
- Crisis plan and emergency contacts
- Follow-up check-ins after graduation
- Open door policy if you need support
Relapse Prevention Toolbox: You’ll leave with concrete skills including:
- Personalized trigger management plan
- Extensive coping strategies for difficult emotions
- Communication scripts for high-risk situations
- List of supportive people to call
- Self-care routines and wellness practices
- Warning signs to watch for
- Action plan if relapse occurs
We’re invested in your success beyond these 12 weeks.
Frequently Asked Questions About PHP
How is PHP different from residential treatment? PHP provides the same intensity of treatment (6-8 hours daily, medical oversight, comprehensive programming) but you return home each evening instead of sleeping at the facility. It’s equally effective for most people and costs less.
Can I work during PHP? No. PHP is a full-time commitment requiring 6-8 hours daily, 5-7 days per week. You cannot maintain employment during PHP. However, this intensive focus is exactly what makes it effective.
What if I relapse during the program? Relapse isn’t failure—it’s information. We’ll adjust your treatment plan, provide additional support, and help you get back on track. We’re here to help you through challenges, not punish you.
Do I need to complete detox before PHP? Yes. PHP is for individuals who are medically stable and past acute withdrawal. If you need detox, we’ll help you find appropriate resources first.
What happens if PHP isn’t intensive enough? If you’re struggling even with full-day programming, we’ll help you transition to residential treatment. Your safety and success matter most.
Do you treat co-occurring mental health disorders? Absolutely. In fact, PHP is ideal for co-occurring disorders because we have psychiatric support and can address both addiction and mental health simultaneously.
Can I attend PHP if I live outside San Diego? Yes! Many clients relocate temporarily for treatment. We offer transitional housing that makes this easy.
